POSITION SUMMARY

The Charge Nurse renders professional nursing care to residents in accordance to the plan of care developed by the interdisciplinary team, the resident, and the responsible party, and in accordance with standards on nursing practice and regulatory agencies. The Charge Nurse is additionally responsible for leading a team of Certified Nursing Assistants (CNAs) and plans, assigns, and directs work. Together with the entire team at Channing House, your role contributes to promoting a healthy and vibrant home for our residents. 

KEY RESPONSIBILITIES

This position will be responsible for, but not limited to the following essential duties and responsibilities: 

Resident Care:

  • Performs resident assessments upon admission and on a continuing basis including discharges. Develops, modifies and implements the resident care plan.
  • Administers, observes, and documents ordered medications, treatments and resident response. Disposes of drugs and narcotics as required and in accordance with pharmacy procedures.
  • Supervises distribution of meal trays to ensure accurate diets are served. Assists in feeding residents.
  • Monitors, records and reports all aspects of resident care including symptoms or changes in residents' conditions, diet and physical activity. Orders diagnostic tests and reports results to physicians.
  • Performs diagnostic tests within scope of practice and reports results to physicians.
  • Promotes resident’s independence and functional abilities through rehabilitative/restorative care. Encourage participation in prescribed therapies, medications, treatments, and activities. 
  • Work closely with Physicians. Informs physician of changes in resident condition, draws blood, prepares patients for and assists with examinations or treatments, transcribes and carries out physician orders.
  • Maintains accurate, detailed reports and records.
  • Maintains the confidentiality of all resident data, under HIPAA.
  • Weekly Summaries and other documentation routinely and as needed.
  • Assists the Director of Nursing (DON) or designee in completing and revising the minimum data set (MDS) if applicable.
  • Attends the required Care Plan Conferences, and update the Care Plans as needed.

Supervisory :

  • Plans, assigns and directs work of Nursing staff. Determines priorities, makes changes to daily assignments as needed, and explains procedures to be followed. 
  • Supervises, delegates duties and works closely with the Certified Nursing Assistants (CNAs) and non-licensed staff. Gives frequent feedback, conducts performance appraisals, rewards and disciplines employees, addresses complaints and resolves problems. Carries out supervisory responsibilities in accordance with applicable laws and organizational policies.
  • Observes skill levels and teaches concepts of nursing care on an informal basis. Ensures the CNAs have appropriate information to provide proper care to residents.

JOB REQUIREMENTS / QUALIFICATIONS 

To perform this job successfully, a person must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

Years of Related Professional Experience

  • 3 years of related work experience in clinical patient care
  • 1-2 years of supervisory experience

Educational/Position Requirements:  

  •  ADN, BSN or MSN degree

Required Licenses/ Certifications/ Background:

  • Valid Registered Nurse License in the state of California
  • Graduate of a Registered Nurse (RN) program or accredited ADN program

Reporting Relationships:

  • Number of Direct Reports (if applicable): 1-4 per shift

Work Environment: 

  • Generally in an indoor setting. 
  • Supervise activities and events outdoors. May be exposed to heat/cold during support of outside activities.
  • The noise level in the work environment is usually low to moderate.

  • Utilize specific vision abilities for viewing work related processes and materials handling.
  • While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to sit and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl.
  • Able to sit at a desk, working on computers and operating phones.
